This factory OEM used Heater/Climate Control Switch from a 2005 Ford Five Hundred (sedan)... Might fit a few newer years but not sure, they ame the Five Hundred til 2007 and then they changed the name to Taurus but not sure if they changed the interior... Look at the pics and see if it looks the same as what your fixing... this unit was tested and worked fine when pulled... It is showed mounted to the bezel but that is NOT included.. If you need that bezel, LMK and I can figure more shipping into the price, I also have the piece that goes around the shifter.. Nice condition with minimale age wear... No cracks, just a few light marks.
